Immunization Information

 

 

All students must have on file with the school a signed Certificate of Immunization Status (CIS) form prior to the start of the school year.   For all vaccine requirements, documentation on the CIS form must include the date (day, month & year) of vaccination or an exemption from immunity based on a medical, religious or personal reason.  Please visit the Washington State Department of Health website for the Recommended Immunization Schedule for Children and Adolescents at: www.doh.wa.gov/cfh/Immunize/schools/default.htm. 

 

 

New School requirements for 2009-2010

 

Kindergarten:

 ·          All children attending kindergarten need 2 doses of the chickenpox (Varicella) vaccine.  Parental documentation of the disease is NO LONGER acceptable.

 First Grade:

 ·          All children entering first grade need 2 doses of the chickenpox (Varicella) vaccine.  Parental documentation of the disease is NO LONGER acceptable.

 Second Grade:

 ·          All children entering second grade need 1 dose of the chickenpox (Varicella) vaccine.  Parental documentation of the disease IS acceptable.

 Sixth Grade:

 ·          All children entering sixth grade need 1 dose of the chickenpox (Varicella) vaccine.  Parental documentation of the disease IS acceptable.

 ·          All children entering the sixth grade need the Tdap (tetanus, diphtheria, acellular pertussis) vaccine IF the student is 11 years old and IF it has been at least five years since the last DTaP, DT or Td vaccine.

 Seventh Grade and Eighth Grade:

 ·          All children entering the seventh or eighth grade need the Tdap (tetanus, diphtheria, acellular pertussis) vaccine IF the student is 11 years old and IF it has been at least five years since the last DTaP, DT or Td vaccine.

 Exclusion Policy (WAC 246-100-166 (6) and (7) and WAC 2246-101-420:

 Students can be excluded from school for the following reasons:

  • Immunizations provided are not complete per the requirements
  • Student does not have a completed and signed CIS form
  • Student is not immunized and a disease exposure occurs
